A historic day for cannabis in Spain
After years of debates, reports and delays, the Council of Ministers has approved today, October 7, 2025, the Royal Decree that regulates the use of medicinal cannabis in Spain .
The measure will allow certain patients to access, under medical prescription, standardized cannabis preparations made in hospital pharmacies.
It is a step forward that many consider historic, although insufficient.
While countries like Germany, Italy, or Portugal already have much more open systems, Spain is finally taking its first real step towards regulating cannabis for therapeutic purposes .
What exactly has been approved?
According to the Ministry of Health, hospitals will be able to prepare and dispense compounded medicinal cannabis formulas to patients with certain pathologies when conventional treatments are not effective.
These prescriptions can only be issued by specialist doctors and dispensed only in hospital pharmacy services .
In this first phase, sales in commercial pharmacies will not be allowed , nor will use by primary care physicians.
Nor does it contemplate self-cultivation , recreational use, or access to cannabis flowers as a final product.
The objective, according to the Government, is to guarantee safety, quality control and medical monitoring. However, this highly restrictive structure greatly limits access for thousands of patients who have been demanding a functional and humane legal framework for years.
For which pathologies can it be prescribed?
Although the Royal Decree does not yet establish a definitive list of indications, the Ministry of Health mentions that it may be used to treat cases such as:
- Spasticity associated with multiple sclerosis
- Refractory epilepsy in children and adults
- Chronic non-cancer pain refractory to other treatments
- Nausea and vomiting resulting from chemotherapy
The AEMPS (Spanish Agency of Medicines and Healthcare Products) will publish the official monographs in the next three months, with details on dosage, conditions of use and medical protocols.
One step forward... but a very short one.
The reaction from patient associations, scientific groups, and the cannabis sector is clear: "Finally, but we're late and with too many limitations."
This decree does not yet represent a real opening of the medicinal cannabis market, nor an immediate improvement for most patients.
Spain lags far behind other European countries , where products with controlled CBD or THC can be purchased with a prescription in community pharmacies or, in some cases, even without one.
Here, the hospital monopoly and the lack of authorized domestic cultivation could mean that access remains slow, expensive, and bureaucratic .
What does this mean for the CBD sector?
For CBD stores, such as Natura Green CBD , this move does not yet change the commercial framework , since the Royal Decree does not affect products with cannabidiol (CBD) without psychoactive effects that are sold as cosmetics or collectibles.
However, it does set a key precedent :
For the first time, the Spanish State officially recognizes the therapeutic value of cannabis .
And that, even if it's only in the hospital setting, opens a door that will never be closed again .
In the medium term, this regulation may generate:
- Greater social acceptance of cannabis as a medicinal plant.
- Boost for new clinical trials in Spain.
- A more favorable environment for future reforms that include CBD, self-cultivation, and controlled sales in pharmacies or specialized stores.
The opportunity: from restriction to well-being
This Royal Decree may seem limited, but it represents a ray of light in a system that for years has turned its back on cannabis and its therapeutic potential .
For years, thousands of patients have been resorting to the black market or importing oils and flowers without control. Now, the legal recognition of medicinal cannabis could be the first step towards complete, transparent, and safe regulation .
The challenge will be not to stay here .
Spain has the potential to become a European leader in research, sustainable production, and responsible regulation of cannabis, if it dares to move beyond the hospital setting .
Conclusion: a beginning, not an end.
Medical cannabis is already a reality in Spain, although for now only within the walls of hospitals .
But it's a start, and every step counts.
